Whether you are a current student or new to Griffith, making an appointment with us is your first step to success.

Supplying documentation
Before your first appointment with us, you'll need to provide recent supporting documentation from a health practitioner that outlines:
- the nature of your disability, injury or medical condition
- the impact your disability, injury or health condition may have on your studies
- the impacts of your medication, if you are taking any.
The documentation must be presented on letterhead and be signed, dated and legible.
Alternatively, you can you can download the Supporting Documentation Form below for your health practitioner to complete.
You'll also need to complete the Release of Personal Information Form to confirm you understand Griffith's Privacy Statement and when and why we may be required to release personal information or may use non-identified data collected through our service.
Our staff maintain confidentiality and privacy with respect to your documentation and information.
Documentation for learning disabilities
If you have a learning disability, please also provide recent results (up to three years old) of comprehensive diagnostic testing from an appropriate practitioner.

Assessments for learning disabilities
If you do not have recent, comprehensive test results for your learning disability, contact your GP or Griffith's Health and Medical Service for a referral to an appropriate practitioner for an assessment.
Griffith’s Psychology Health Clinics at Mt Gravatt and Gold Coast campuses can also provide assessments for learning disabilities.
How to make an appointment
Confidentiality and disclosure
In accordance with the University's Privacy Statement, Student Disability and Accessibility will not discuss or release confidential information about you without your written permission to do so, with the following exceptions:
- when required by law
- where there is significant risk to you or another person
- where disclosure is inherent in the nature of the contact (for example, you are seeking assistance gaining employment or you are seeking assistance with arranging disabilities support).
We also understand that in some circumstances you may require a partner, parent/guardian, or carer to contact us on your behalf.
You can discuss the process for this at your initial interview.
Disclosing your disability to others at the university is a personal decision and completely up to you.
